Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. With over 100 locations throughout Southeastern Pennsyl- vania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices.
We are seeking qualified candidates interested in joining our multidisciplinary Intensive Case Management (ICM) Team at our Wilmington, Delaware location. This position will work in collaboration with Nurses, Case Managers and Clinicians in a community based setting.
PURPOSE:
▶ Psychiatric assessment, evaluation and diagnosis of participants.
▶ Attend and participate in participants’ case conferences as a co-treatment team leader; oversee the formulation and monitoring of treatment plans.
▶ Clinically supervise the psychiatric treatment of all participants by monitoring and ensuring the provisions of an acceptable level of clinical practice.
QUALIFICATIONS:
▶ Board Certified Licensed Psychiatrist in the state of Delaware.
▶ Prior experience in servicing community mental health facilities and their psychiatric needs.
▶ Comprehensive knowledge of psychiatric, clinical and psychosocial rehabilitation practices.
▶ Excellent communication skills both oral and written.
▶ Strong interpersonal skills.
▶ Very strong organizational and analytical skills.
▶ Detail-oriented.
