We can help you help them.
The Strong Bones Program at Christiana Care helps you and your patients break the bone fragility cycle. This program for patients who have risk factors for fragility fractures is designed to:
• Reducefracturesbyprovidingafractureliaisonservice.
• Coordinate bone health care.
• Ease the burden on primary care providers by delivering focused
care for bone fragility.
• Provide quality DXA and fracture risk assessment.
Our doctors and nurse practitioners from a variety of specialties, including endocrinology, orthopaedics, rheumatology, sports medicine and women’s health, can assist in this aspect of your patients’ care. We’ll pair each of your patients with a nurse navigator who will:
• Schedule an initial assessment and DXA if needed.
• Provideeducationonriskfactors,calciumandvitaminD.
• Recommend exercises and safety precautions, and discuss
medication options.
