Methods: This is the second Plan- Do-Study-Act (PDSA) cycle of a
       project, a root-cause analysis found a relatively low ordering rate (39.66%) vs. the PFT completion rate (75.7%). The      education. The current PDSA cycle focused on educating patients with outreach letters. An electronic medical record program, Health E-Registries, was used to review medical charts to identify target patients. Letters were sent to these patients to encourage them to request the test. Residents were instructed to inform our team when patients requested PFTs. Health E-Registries was reviewed three months following the intervention.
Results: Outreach letters were sent to 197 patients from the clinic. Three patients responded and requested PFTs, a 1.5% success rate for this second PDSA cycle intervention.
Conclusions: The 1.5% success rate
of this intervention was below the 10% target. While in-reach and outreach       to peak and then tail off; hence, they are not always sustainable. The next step in the quality improvement initiative is to work with the PFT lab to decrease the time between order and appointment to eliminate other limiting factors.
